On July 2, 1935, Salisbury was admitted to Patton State Hospital for the Insane in San Bernardino, California, his name unknown, his occupation a hotel clerk. A month later, on August 6th he fell, hitting his head and fracturing his skull. He lay in the morgue for two days before his sister, Adelaide Cash Bosche, identified him giving his real name as Orr S. Cash.
In February 1932, he returned to Southern California, at San Pedro, from Ensenada, Mexico, aboard the S.S. Ruth Alexander, and gave his U.S. address as 6326 Lexington Hollywood.
His last movie was in 1930 after which he disappeared from the Hollywood scene.
In June 1928, he returned to the U.S. at the Port of San Francisco, from Kobe, Japan, aboard the S.S. Taiyo Maru, and gave his U.S. address as Hemet, California.
By the late 1920s, the aging actor was less in demand and he struggled to find parts.
When he registered for the draft of World War I, in late 1918, Salisbury and his mother were living in the Mountain View Inn at 5956 Hollywood Boulevard in Hollywood.
His most famous part was that of Allesandro in the film "Ramona" (1916).
By 1914, he left New York for Hollywood to work in films and became well-known, playing opposite such actors as Douglas Fairbanks, Sr. , Lon Chaney, and Ruth Clifford eventually becoming a movie idol. He was six feet tall and ruggedly handsome, making him a great choice for western films.
In the late 1910s he bought a 40-acre (160,000 m) citrus ranch near Hemet, and, between pictures, it was his habit to drive out and drop in unexpectedly on the Native American overseer and his family who lived on the place and worked in the groves.
He performed on Broadway between 1903 and 1906.
Monroe Salisbury was born Orange Salisbury Cash, the son of David Cash and Ellen Louise Salisbury Cash, and grandson of Aaron Cash and Ann Roat Cash of Evans, Erie County, New York. He grew up with two sisters, Adelaide Mary Cash and Anna Louise Cash. By 1900, he was a working actor supporting his widowed mother and living in Providence, Rhode Island.
Salisbury was a matinee idol. He began his acting career on the stage in 1898, appearing in numerous romantic leads. He also appeared in five Broadway productions. He was in 42 silent movies between 1914 and 1922, working frequently with director Cecil B. DeMille. Salisbury, who appeared in several western films, also appeared in two talkies, in 1929 and 1930. After his career was at an end, Salisbury died at a mental hospital from a fractured skull sustained during a fall.

Orr's two elder sisters were Adelaide Mary Cash (1864–1956), who married John Casper Bosche (1861–1929), and Anna Louise Cash (1868–1951), who married Edward Wright Clarke (1873-1938).
