He attended the National College of Art and Design in Dublin, where he studied acting. He began his career in the theatre, appearing in productions such as The Plough and the Stars and The Playboy of the Western World. Gaffney made his film debut in the 1991 musical comedy The Commitments, playing the role of Jimmy Rabbitte. He went on to appear in films such as The General (1998), The Wind That Shakes the Barley (2006), and The Guard (2011). He has also appeared in television series such as The Tudors (2007-2008) and The Borgias (2011-2013). Gaffney has been married to his wife, actress and director Mary McEvoy, since 1985. He won the League of Ireland Cup and made 4 appearances in European competition while at Glenmalure Park. He was an unused substitute in the 1978 FAI Cup Final.
