He attended the Danish National School of Theatre and Contemporary Dance in Copenhagen, graduating in 1979. Nielsen's first major role was in the 1987 film The Seventh Seal, directed by Ingmar Bergman. He went on to appear in several other films, including The Best Intentions (1992), The Kingdom (1994), and The Idiots (1998). Nielsen has also directed several films, including the short film The Last Supper (1996) and the feature film The Island on Bird Street (1997).
